Recording incident investigations

Lobel Services completes a Services Incident Investigation Report (on the proforma worksheet is shown below) in the event of:

• • any injury involving medical attention or off-site treatment;

  • any incidents involving a near miss, property/plant damage; or

  • any injury to the public or any damage to the environment.

The Principal is informed immediately in the event of any of the above situations. Following discussions with the Principal, a decision will be made as to who will conduct the incident investigation. The Principal will be provided with a copy of the completed Services Incident Investigation Report.
